Re: Potential Novel Coronavirus (COVID-19) Exposure

To whom it may concern:

Your health and well-being are of utmost importance to us. Please read this carefully.

We are writing to inform you as a patient/visitor of Urology Centers of Alabama between the dates of (March 16, 2020 and March 30, 2020) you may have come in contact with a person at our practice who has been diagnosed with Novel Coronavirus, also known as COVID-19.

COVID-19 is a respiratory virus disease that is spread mostly through the respiratory secretion (or droplet) route, and less commonly through direct contact or the air. Because of this, for 14 days after your first date of potential exposure, the public health department and Urology Centers of Alabama strongly recommend that you do the following:

1. Watch for symptoms
   1. Reported illnesses have ranged from mild symptoms to severe illness for confirmed coronavirus
   2. These symptoms may appear 2-14 days after exposure:
      1. Fever
      2. Cough
      3. Shortness of breath

2. Contact your provider (call ahead) should you develop any of these symptoms. This list is not all inclusive. Please consult your medical provider for any other symptoms that are severe or

4. If you do not have symptoms you may:
   1. Continue with your normal routine of daily
   2. Practice good hygiene, including washing your hands with soap and water or alcohol hand
   3. Clean and disinfect your home environment on a routine
   4. Practice ‘social distancing’ as outlined by local health announcements for your area, but at a minimum, as defined as no groups of more than 10 people; keeping a 6-foot distance from others; and avoiding going into areas with large numbers of
   5. Wear a facemask if you feel like you are becoming sick, and contact your provider

If you develop any symptoms, a discussion with your medical provider and the public health department will determine if you need to be tested for COVID-19. If you are symptomatic and experiencing a medical emergency call 911 and inform them of your exposure.

Additional facts to consider:

 Older adults and people of any age who have serious underlying medical conditions may be at higher risk for more serious complications from COVID-19. The people who may be at a higher risk of getting very sick from this illness, include:

• Older adults (>65)
• People who have serious underlying medical conditions like:
  • Heart disease
  • Diabetes
  • Lung disease
  • Hypertension (high-blood pressure)
